Default Immigrant Visa Interview reschedule risks

In May 2005 I was given an appointment for an immigrant visa interview at the Montreal US consulate (Canada) for an employment-based immigrant visa (E3). However, I was not able to attend and sent them right away a letter requesting to re-schedule such interview for December 2005 (I was working in a project in Australia at the time and had a ticket to return to Canada for x-mas).

Months passed without any news about my re-scheduling request and in May 2006 I decided to call the immigrant visa section of the Consulate about it. They told me that my case was about to be terminated by lack of contact on my part!!

I told them about my letter requesting re-scheduling for December 2005, which they apparently did not have on their system, but managed to locate within my folder after 30 minutes of heated discussions over the phone

They apologized for misplacing my letter and, in order to continue my application, asked me to immediately re-submit a new DS-203, which I did. I called them again and they confirmed that they did received my new DS-203 on June 10th, 2006. I was told that I would be re-scheduled for a new interview once they received confirmation from the NVSC

I hope that somebody would be able to answer the following questions:

1. If I already was given a visa number and was scheduled for interview in 2005, how come I need to submit a DS-203 again? Do I need a new visa number?? I thought I had one already...

2. My cut-off date is December 2002 and my first interview had been scheduled about one month after I submitted the first DS-230 to the Consulate (which I did in April 2005). How come now is taking more than three months??

3. What type of communication the consulate needs to establish with the NVSC after receiving my new completed DS-230 in order to re-schedule my immigrant visa interview??
